Tummy Tuck or Liposuction? What is the Difference Between Tummy Tuck & Liposuction Surgery?

Liposuction refers to an operation that removes adipose tissue with the use of cannulas to improve body contour. Abdominoplasty is the repair of the abdominal wall (abdominal muscle plication) as well as the excision of extra / damaged skin for improved abdominal contour and skin tightening.

Liposuction procedure varies in duration depending upon how much work needs to be performed. Liposuction is an outpatient procedure with minimal recovery time. Incisions for the procedure are generally one quarter inch in length and are placed in inconspicuous skin folds. Liposuction will not change the appearance of the skin. The operation does not involve any skin removal or re approximation of abdominal wall muscles or skin tightening. The best candidates for liposuction surgery of the abdominal wall are patients with good quality skin elasticity and a moderate degree of diet and exercise resistant adipose tissue superficial to the abdominal wall muscle layer. Patients who are significantly overweight, have significant abdominal wall laxity, and/or have lost a significant amount of skin elasticity are not generally good candidates. It is important to realize that suction lipectomy is not a skin tightening method. When a patient is bothered by loose or extra skin the best procedure will be an abdominoplasty. This will remove the excess skin and also provide the best contour of the abdomen when dealing with loose, dimpled, wrinkled, strechmarks, and abdominal laxity.

Abdominoplasty surgical duration can vary from 3 1/2 – 5 hours depending on the patient and is also performed as an outpatient surgical procedure. Incision site for the abdominoplasty can be from hip to hip (for a standard tummy tuck) or extend into the back (for an extended abdominoplasty).

The “ideal” patient for tummy tuck surgery is one who has completed pregnancies, is psycho socially/emotionally/financially stable, has an excellent social support system surrounding him/her, is capable of arranging enough recovery time, does not smoke and who has reached a long-term stable weight. The tummy tuck operation involves removal of excess skin/adipose tissue of the lower abdominal wall and re approximation of the abdominal wall muscles that may have spread with pregnancies and/or weight gain/loss. Liposuctioning is often used as a tool during the tummy tuck procedure.
 
 
Liposuction risks include:

• Skin re-draping problems: liposuction is not an exact science and so the skin may not re-drape exactly after surgery.
• Asymmetry: differences between the areas treated may necessitate further surgery.
• Scarring: small scars may form where the incisions are made to allow for the liposuctin cannula. Scar revision may be necessary after everything is healed.
• Infection: rare and usually treatable with antibiotics.
• Bleeding: rare but may require surgical intervention.
• Deep Venous-Thrombosis/ Pulmonary Embolism: blood clots that form in the legs that may travel to the lungs causing difficulty with respiration and requiring hospitalization.
• Prevention is key using compression stockings and early ambulation.
• Need for revisionary surgery
 
 
Risk associated with Tummy tuck

There are risks associated with tummy tuck surgery; the majority of serious complications that may occur with tummy tuck surgery occur within the first 2 weeks. These may include deep venous thrombosis, pulmonary embolism, pneumonia, infection, bleeding, and even death. Fortunately, the potential for these types of major complications are very small, assuming
careful selection of a board certified plastic surgeon, board certified anesthesiologist, and fully accredited surgery facility. Other types of complications that are more likely include superficial incision line healing problems ( wound healing problems), seromas, abdominal wall asymmetry, abnormal scarring, unsatisfactory cosmetic results. Careful selection of plastic surgeon will help minimize potential risks associated with umbilical hernia repair as well.

Breast Implant Removal Options

Patients with breast implants may choose to remove them for various reasons, such as lifestyle changes, aesthetic reasons, and/or potential health complications they feel are stemming from their implants. There are three types of breast implant removal surgeries….

• Straightforward Breast Implant Removal:
This procedure consists of removing just the breast implants. This procedure is ideal for patients who are not experiencing any complications and would like to remove their implants for aesthetic reasons or lifestyle changes that resulted in them no longer wanting their implants. Straightforward implant removal is a fairly quick and simple procedure with a relatively speedy recovery. Another reason why patients would choose to just remove the implant rather than remove both the implant and the capsule is if the capsule is thin and delicate. Removal of a thin capsule can result in bleeding and possibly removal of the remaining breast tissue. If the capsule is not thick, calcified, or abnormal in any way, some believe removal does not need to be an option.

• Implant Removal With Capsulectomy:

This procedure consists of removing the breast implant, as well as removing the capsule surrounding the implant. This procedure can be a partial capsulectomy or total capsulectomy. A total capsulectomy involves removing the entire breast capsule from around the implant. A partial capsulectomy involves removing only part of the capsule. Since the capsule does not disappear on its own after the implant is removed, some patients opt to proceed with a capsulectomy. Sometimes during a partial capsulectomy when the entire capsule cannot be removed for whatever reason, the remaining capsule will be cauterized to sterilize the area and promote healing.

Implant removal with capsulectomy can expose patients to additional risks, such as bleeding, size loss, contour irregularities and other serious complications. In other words, any maneuver performed during surgery exposes patients to additional risk (morbidity). For example, attempting to remove very thin capsule densely adherent to the patient’s rib cage may expose the patient to significant bleeding and/or entrance into the thoracic cavity.

• Implant Removal En Bloc:
En Bloc breast implant removal refers to a procedure where the breast implant and the surrounding capsule is removed as a single unit. The procedures is performed such that the contents within the breast implant capsule do not come into contact with the surrounding tissues. Generally, the procedure requires a longer incision (6-7 centimeter) usually in the inframammary fold, in my experience. The procedure is more successful when capsules are thickened as opposed to when patients have very thin (normal) capsules. Also, sometimes complete capsulectomy is not safely possible; for example, breast implants placed in the sub muscular position may have a capsule that can be densely adherent to the patient’s rib cage. Removal of this posterior capsule can be potentially dangerous.

Generally speaking, unless the breast implant capsules have thickened (and/or are otherwise symptomatic), are associated with the ruptured silicone gel breast implants, or if the patient has concerns about “medical conditions” related to the breast implants, capsulectomy is not universally necessary. For these patients, En Bloc removal of breast implants is a good procedure.

What To Expect After The Surgery

Rest in bed is the order of the day, along with plenty of fluids and no aggravation. You will be permitted to shower, perhaps as quickly as two to three days post-op. Also, the chest will feel somewhat sore for the first few days. This should abate very quickly, and by the end of the first week you should be very comfortable.

  • Pectoralis implants can be used to augment the male chest wall. The procedure takes 2 hours and is performed under general anesthesia. Generally, the procedure is done through an incision in the axillary (armpit) area. The silicone implants are placed underneath the pectoralis muscle.

  • Chest Wall Reconstruction
  • Pectus Excavatum, also known as cobbler’s chest, sunken chest, funnel chest or simply a dent in the chest, is the most common congenital chest wall deformity, in which several ribs and the sternum grow abnormally. This produces a caved-in or sunken appearance of the anterior chest wall. This operation may serve to correct a congenital deformity or tear of the pectoralis muscle. The procedure takes 2 hours and is performed under general anesthesia. Generally, the procedure is done through an incision in the axillary (armpit) area. The silicone implants are placed underneath the pectoralis muscle.

Treatment of congenital symmastia can be challenging and could benefit from liposuction of the between the breasts (sternum area). The use of compression gauze and a “t-bra” will help put pressure on that area for improvement in contour.

In general, it may take several procedures to achieve a separation. If the patient with congenital symmastia is also interested in breast implants, there is an option to have breast implants placed as well as the liposuction of the cleavage area in one surgery and then after healing and recovery, proceed with a 2nd surgery to perform internal sutures (capsulorrhaphy) for better definition and contour of the cleavage area. Patients with congenital symmastia must have realistic expectations.
 


Patient / Surgical Example:

This 26 year old teacher from out of state had been thinking about breast surgery for over 20 years. She and her husband had been searching for the best plastic surgeon who had the confidence to improve her situation. She previously had the Nuss procedure for her pectus excavatum and had a metal bar place across her chest. She also has symmastia with webbing and excess skin of the cleavage area.
 

 

 

The markings below are a “road map” for Dr. Pousti through out the procedure. The “green” area is the skin tenting and the area where liposuction will be performed to achieve more contour. The “red arrows” indicate where the patient wants more fullness from the breast implants.
 

 

Dr. Pousti injects tumescent fluid prior to performing liposuction to help with anesthesia and control bleeding during the procedure.
 

 

The patient was very unhappy about the skin webbing across the bottom of her chest. She already had a scar across her chest from the Nuss procedure so Dr. Pousti recommended using the same scar and excising the excess skin. The patient was very happy with that option. Below you will see the area where the excess skin was removed completely prior to closure.

Description

Sometimes with saline breast implants, patients desire to reduce the size of their breast implants or wish to gain greater symmetry after a period of time. Others wish to get closer to their original breast size without undergoing surgical procedures again.

Dr. Pousti is skilled at saline implant deflation and is able to care for many cases from inside the office as opposed to patients going in for surgery. The implant(s) are deflated by needle aspiration through a series of saline removal steps. Patients are comfortable through the entire process.

Lateral Displacement

Lateral Displacement

Lateral Displacement involves lateral (toward the armpit) migration of the breast implants. This causes the nipple-areola complexes to appear closer together.

The support of the implant by the skin alone is not always enough support to prevent lateral migration of the implants. To add another layer of support laterally, Strattice is placed on the sides of the breasts between the skin and the implant to help prevent lateral displacement.

The Surgical Procedure
The operation time varies depending on what exactly would need to be performed. We will review this with you after you have been examined by Dr. Pousti.

For correction of lateral displacement, the procedure can take from 1-3 hours depending on how much work is involved. Board Certified Plastic Surgeon, Dr. Pousti takes his time in the operating room to make sure that he does what he can to achieve the patient’s results. Usually an internal suturing technique is used to correct the pocket and therefore the implant placement on the chest.

What To Expect After Surgery
Pain is rarely strong, more commonly being a degree of discomfort. Swelling occurs but usually begins to subside by the third or fourth day. Some degree of swelling may persist for longer periods. A well-fitted bra is worn day and night for three weeks. While there is probably seldom interference with future breast feeding, women are cautioned that it may not be possible to breast feed in the future.

The surgical bra is used to stabilize the area after the repairs are made.

Resumption of Physical Activities
Driving may be resumed in 1-2 weeks.
Non-contact sports in 3 weeks.

I think that I have a pretty high tolerance for pain so I am going to say that my recovery was fairly easy. Day one I slept quite a bit. I can’t stress how important it is to stay on top of your pain medication and no lifting of any kind. After a week I was back to driving but I was also off pain medication. Back to work after 2 weeks. Since the granny makeover is comprised of multiple procedures, and every granny makeover is different for each patient’s needs, the actual surgeries and surgery lengths vary. I started my Granny makeover by scheduling a consult with Dr Pousti in May, and then scheduled my breast augmentation and lift for the end of August. Resumption of Physical Activities

Depending on the procedures performed bed restriction can vary from 1 to 3 days, mild activity may be restricted from 1 to 2 weeks, and it can take anywhere from 4 to 6 weeks for a full recovery and the ability to return to strenuous activities.
