Some patients choose to change their breast size after undergoing breast augmentation surgery. They may have chosen a larger size when they were younger and are now ready to have a more natural look so they consider removing their breast implants and replacing them with smaller breast implants. Each patient differs with the procedure that will be needed to achieve their goals. Sometimes the surgery is as simple as removal of the current breast implants and replacement with smaller breast implants. Sometimes, when reduction in size is wanted, capsulorrhaphy (internal sutures) and/or a mastopexy (breast lift) is also needed to remove or reconstruct excess skin that was stretched due to the larger implants or for better placement of the implants on the chest wall.

The Surgical Procedure of Downsizing Breast Implants with Mastopexy (breast lift) or Capsulorrhaphy (internal Sutures):
The procedure takes approximately 2-3 hours. Breast implant reduction/ breast lift surgery is usually performed in our outpatient surgical facility or in a hospital, depending on the size of the operation and the health status of the patient. New breast reduction techniques are now available that offer more limited scars. The choice of technique used depends upon patient age, goals and anatomy. The capsulorrhaphy procedure, also known as pocket correction or use of internal sutures, involves closing a portion of the breast pocket to correct displacement of the implant that may happen when downsizing.

Patient Testimonials:

“When I was eighteen I underwent breast augmentation because I was insecure about my body. I felt that breast implants would give me the confidence I lacked and ultimately make me happy. Bra shopping was a nightmare and when wearing a sports bra or bikini I was flat as a pancake. To my eighteen year old self I felt I was lacking something. Now eight years have passed and I am not the same person I was at eighteen. I am still young, but I feel fortunate to have discovered who I am and what I believe in, and what ultimately matters to me. I have been married for four years to a man who loves me for me. And just six months ago we had a beautiful baby girl. I had been thinking of having my implants removed for the last three years and being pregnant with them and breast feeding solidified my decision.
I had my implants removed three weeks ago and I feel liberated! Not only was my husband supportive of my decision, he also was encouraging throughout my appointments, surgery and the outcome of my body without implants.
I would encourage any woman who is toying with the idea of implant removal to do your research and come visit Dr.Pousti He is honest, professional and more than certified.”

Pregnancy After Tummy Tuck

Having a tummy tuck procedure is a life changing experience to repair a woman’s abdomen after childbirth or extreme weight loss. Some patient’s experience a planned or unplanned pregnancy after their tummy tuck procedure and have several concerns regarding their growing abdomen and unborn child. Experiencing pregnancy after a tummy tuck still allows for a successful pregnancy and does not pose any harm to the unborn child, however it may result in the need for an abdominoplasty revision after pregnancy. Further surgery may be necessary after pregnancy if excess skin is present or if muscle diastasis (separation of the abdominal muscles) occurs after childbirth. Plastic surgery does not guarantee permanent results and pregnancy after plastic surgery can change the results of your previous surgery. Many women have come to Dr. Pousti for an abdominoplasty surgery and some have experienced childbirth after their procedure. Most of the women are satisfied with their previous surgical results after pregnancy while others prefer to have a revisionary tummy tuck procedure performed. Many factors such as weight gain, stretch marks, skin laxity, muscle separation, and personal desires will determine if revisionary surgery is necessary post partum.

Description

Breast asymmetry is a condition where one breast is noticeably larger than the other. Some cases of breast asymmetry are more severe than others, but there are several surgical options to promote better symmetry. In some cases, the breast are augmented using different size implants to correct asymmetry. A breast lift or breast reduction can also be performed to achieve better symmetry. Dr. Pousti will determine which procedure will benefit each individual patient based on her concerns.

Procedure
The breast asymmetry correction procedure takes approximately two hours depending on the treatment plan. Placement of the implant is usually centered on the nipple-areola. Often, placement above or below the muscle is decided on an individual basis. Likewise, the incisions used to place the implant vary with patient preference/anatomy and degree of asymmetry present. The surgery is performed in a state of the art outpatient surgical center under general anesthesia. The size and type of implant to be used, along with their relative advantages will be discussed with your doctor at the pre-operative appointment.

Before Breast Augmentation/ Right Side Benelli Breast Lift

Markings are performed prior to surgery and the surgical plan is mapped out in detail.

Sizer implants are used to determine the appropriate implant size needed to correct the breast asymmetry.

A benelli lift was performed on the left breast and a slightly larger implant was used on the right breast to achieve better symmetry.

Patient Testimonial:

“I have just recently had my breast augmentation with Dr. Pousti and his wonderful staff and it has by far exceeded my every expectation and more. My whole life, I had dealt with asymmetric breasts and it was incredibly hard for me. I couldn’t wear a lot of shirts, bathing suits were a joke and I couldn’t go to the gym without wearing 3 shirts comfortably. I had been battling this since childhood and learned to hide it pretty well, but it was so frustrating being so young, in my early 20’s, and being so active. I didn’t ever feel comfortable in my own skin… until I made the decision to go ahead with my surgery.

I had one consultation with another doctor who seemed nice, but I wasn’t confident he could perform the exact surgery that I needed because of the complications in the lift and correcting the size factor. After I had done all of my research, I met with Dr. Pousti and his staff. After my first visit, I was completely confident in him. He was so kind hearted and easy to talk to; plus it was so easy to see how incredibly experienced he was in this type of surgery. The staff was so knowledgeable in answering all of my questions and I always felt and knew I was in good hands. It was from that point on that changed my whole outlook of myself. From A to Z, Dr. Pousti and his staff made this surgery for me absolutely remarkable.

The surgery day was a breeze and the recovery was so much easier than I imagined. I was up and back to my normal self in no time. The most amazing and unforgettable day was when I had my bandages removed on my first post-op appointment. I hadn’t been able to really see the results yet when Kara removed everything and I got to see everything, I just broke down. Not only was my asymmetry corrected, but also they were absolutely amazing looking! I finally felt like a young woman and I was so happy. Dr. Pousti is truly an artist and is such an amazing surgeon; I wouldn’t recommend any other physician after my experience there. I owe all the gratitude in the world to Dr. Pousti for his amazing work and literally could not be happier; he did exactly what I asked for and more.

THANK YOU DR. POUSTI!!”

What To Expect After Surgery
Rest in bed is the order of the day, along with plenty of fluids and no aggravation. During the first week, a brassiere will be fitted and you will be permitted to shower usually within 2-3 days, but longer if a breast lift/breast reduction is performed. Initially, the breasts will appear to be slightly higher than normal. Over three to six weeks the implant will descend to a more natural position. Also, the chest will feel somewhat sore for the first few days. This should abate very quickly, and by the end of the first week you should be very comfortable.

Resumption of Physical Activities
After one week of resting at home, “normal activities” such as driving, working, socializing may be resumed, except NO exercise or sports for four to six weeks. It is very important to follow these guidelines, since it is possible to shift the implants to an abnormal position if exercise or physical activity is resumed too soon.

1. No driving for 3 weeks post tummy tuck surgery or until off narcotic pain medication completely.

2. Please take the medications as directed. Antibiotic medication is for 2 weeks, pain, nausea and Valium medication are taken as needed. Never on an empty stomach and always space out the medications.

3. Constipation is very common due to the pain medication and anesthesia, please use Colace over the counter or any laxative that has worked with you in the past. You may start taking something to help prevent constipation 2-3 days post surgery.

4. If you develop a rash or think you are having an allergic reaction to the medication please call our office immediately. Sometimes Benadryl will help.

5. No exercises for 6-8weeks post surgery

6. No showers for 6 weeks post surgery. No ocean water or pool water for 8 weeks post surgery or until the incision is fully healed.

7. No tanning or sun exposure to the incision until the incision has healed completely.

8. Please do not put scar creams or ointments on the incision immediately after surgery, you may irritate the area or cause an allergic reaction.

9. Patients need to be in the hunched over position for 4 weeks.

10. Please use a walker during your 4-week recovery. Try to borrow a walker from a friend or family member or you may rent or buy a walker.

11. Remember to move your feet back and fourth for 2 weeks post surgery.

12. Remember to do the deep breathing exercises for 2 weeks post surgery.

13. It is a good idea to have a recliner to help keep in that recovery position for sleeping. Often times you need a lot of soft pillows to help keep you comfortable.

14. After surgery you will have lots of dressings, binder, drains, pain pump, and compression stockings provided to you. Please leave everything on until we see you, which is usually 7 days post surgery.

15. Pain pump is removed 1-week post surgery.

16. Drains are removed 2-4 weeks post surgery if they are ready. Drainage output of the drains needs to be below 20cc within a whole 24hr period.

17. Patients must wear their surgical binder for a total of 2-4 weeks or until drains are removed and may start wearing a spanx garment.

18. After drains are removed patients need to wear a spanx garment/body suit to help with the swelling for 4 weeks. The spanx garment needs to cover the whole abdomen area. You may find them through our office for the amount of $82, add $10 for shipping or you may also try your nearest department store Khols,Walmart, Targel etc……

19. After drains are removed we want to make sure that there is no fluid build up “Seroma”, please make sure to call the office if you feel you have developed fluid. Sometimes fevers, nausea will be a side affect or the abdomen area may have swelling and you can feel fluid move. Almost like a waterbed affect.

20. You may expect to be swollen for a few months. Sometimes the most swelling will be at about 2 months post surgery and note that swelling will travel south so the pubic area may look swollen too.

21. Bruising can take a few weeks before it goes away, arnica may help to minimize bruising.

22. Patients may start applying scar creams to the incisions after 3 weeks if the incision line if it looks good and there are no open wounds or scabs.

23. For Scar treatment most patients will use Hybrisil Ointment which you may get from our office for $90. Add $10 for shipping. Hybrisil is prescription strength medication that helps prevent keloiding and redness, and is normally used twice a day for 2 months. You may also use anything that has worked for you in the past. Scars will look their worst and angry looking closer to 2 months post surgery.

24. It’s very normal to have some drainage or seapage from the incision line during the healing period.

25. Incision lines and final results may take up to about a year. Sometimes sutures take time to dissolve, and sometimes you may have them reach the surface. This is normal and you may gently trim them.

Description

Juvenile Breast Reduction is designed to improve the appearance and symptoms of overly large breasts in young teens. Breast hypertrophy can cause both physical and psychosocial consequences as the breasts are too large for the teen’s body frame. The teen may experience different degrees of back, shoulder, and neck discomfort associated with large breast and may seek the care of a chiropractor to temporarily alleviate some of the pain. Simple juvenile activities such as exersize or shopping are bothersome to young teens experiencing breast hypertrophy. An ideal candidate is at or near their ideal goal weight and must be willing to accept scars resulting from breast reduction surgery in exchange for the improvement in size, contour, and position of the breasts. While breast reduction surgery is one of the most patient pleasing operations performed, the need for further surgery may be necessary in the future if the breasts were to grow large again, for example after pregnancy.

This is an actual patient of Dr. Pousti.

The Procedure

During juvenile breast reduction, the excess breast tissue, adipose tissue, and breast skin are removed. This procedure both reduces and lifts the breast to a more pleasing position on the breast mound, improving neck, back, and shoulder discomfort. With the reduced discomfort, teens find it easier to perform their daily activities such as exercise and finding clothing as well as feeling comfortable in clothes is no longer an issue.

Patient’s Testimonial:

I’m 16yrs old and just had the best experience of my teenage life. A breast reduction! Most would say that 16yrs old is way too young to make such a permanent decision about one’s body. For me the decision was warranted. At the age of 13 my breast had already reached the size of 28D and I my body frame was a size zero. Talk about disproportion. By the age 14yrs I had become so uncomfortable with the heaviness dragging on my neck and back that I had to give up gymnastics, horseback riding and tap dance-all things that I loved. I was never one to be uncomfortable with my body, I believe people should love who they are, but having such big breast on my small frame caused me not only physical pain but emotional pain too. I didn’t like the way people looked at me, I didn’t like the way clothes fit, I didn’t like the attention my breast unintentionally brought me. Things like prom dress shopping and getting fitted for a costume for the school play, things that should be fun and memorable made me miserable and embarrassed. I didn’t dare wear a bathing suit or tight fitting t-shirt, those things and the attention they brought just didn’t fit my personality. By age 16yrs I was size 30F and miserable (l say F-s plus because l was busting out of that size but refused to go bigger)

This surgery is the best thing that has happened to me. I knew I wanted a breast reduction by the time I was thirteen and l was bound and determined to get one. It took several years of coaxing from my mom and my team of doctors-pediatrician, physical therapist, chiropractor and especially Dr. Pousti…he’s a hero in my book. I felt he understood my pain and wanted the best for me. I’m so glad that Dr. Pousti and his team are the ones that got my insurance to approve this surgery. They made me amazingly comfortable, I wasn’t scared at all. I woke up from the surgery instantly feeling the relief in my neck, back and shoulders that l had dreamed of for over three years. I’m now extremely happy with my body, the results are amazing. I couldn’t have asked for a better doctor.

It’s only been three weeks since my surgery and I honestly feel like a whole new person. Shopping for clothes is super fun…I can buy bras off the rack! I can wear strapless! I love my new 30B’s. They are perfect for me and l’m a very happy confident person because of how much Dr. Pousti and his Team cared. They Rock!

Sincerely with Love

Possible Risks and Complications of Breast Implant Size Reduction/ Breast Lifting

Asymmetry: Almost everyone’s breasts are different to begin with and Dr. Pousti will do his best to make them as symmetric as possible — we can never guarantee that they will be perfect.

Bleeding: It is possible, though unusual to experience a bleeding episode during or after surgery. If post-operative bleeding does occur, it may require emergency treatment to drain accumulated blood. It is important after surgery, especially in the first 2 weeks, to avoid exercise and raising your heart rate and blood pressure. Also avoid aspirin, ibuprofen and many other medications that may affect your ability to clot your blood.

Loss of Blood Supply to the Nipple-Areola Complex: Limited incision techniques are used, however because of the invasive incisions and relocation of the nipple/areola complex a blood compromise can occur.

Mal-Positiong of the Nipple – Areola Complex: Depending on implant placement after surgery, the nipple – areola complex may not be in the ideal location and further surgery may be necessary.

Tissue Necrosis: If a lack of oxygen to the tissue is present after surgery, the tissue will turn purple or blue and if prolonged, tissue damage will occur.

Infection: This is very rare but can be taken care of with antibiotics if it occurs.

Loss of nipple/areola sensation: Numbness may occur temporarily or permanently from traumatizing sensory nerves in the breast during the procedure.

Inability to breast feed: This is not exact because some patients cannot breast feed prior to the surgery.

Inability to predict exact cup size: We will discuss your goals and do our best to achieve them.

Breast Asymmetry: The breasts will be “sisters not twins” and symmetry is never promised. Breasts are asymmetrical before surgery and will most likely be asymmetrical following surgery. Differences in terms of breast and nipple shape, size, or symmetry may also occur after surgery.

Delayed Healing: Wound disruption or delayed healing is possible. Some areas of the breast skin or nipple region may not heal normally and may take a long time to heal. Areas of skin or nipple tissue die. This may require frequent dressing changes or further surgery to remove the non–healed tissue. Smokers have a greater risk of skin loss and wound healing.

Spreading of the Areola / Scar: All surgery leaves scars, some more visible than others. Abnormal scars may occur within the skin and deeper tissues and may be unattractive and/or a different color than the surrounding tissue. Spreading of the areola may also occur.

Need for Revisionary surgery: It is possible that you will need to have one or more re-operations over the course of your life because of local complications from breast implants. Reasons for re-operations could include any of the potential

Tissues all need oxygen and other nutrients in order to stay healthy. If the oxygen is compromised it is not getting to the tissues in a high enough concentration, the tissues will turn blue or purple and become ischemic (which means lack of oxygen). If this lack of oxygen goes on for a long enough time period, necrosis or tissue death may happen.

Possible Risks and Compications of Breast Lift with Breast Augmentation

Leakage: Leakage of the implant sometimes happens. It may be because of trauma to the area or just for no reason. This can be fixed by returning to the operating room and replacing the implant.

Asymmetry: Almost everyone’s breasts are different to begin with and Dr. Pousti will do his best to make them as symmetric as possible — we can never guarantee that they will be perfect.

Capsular Contracture: It is a normal process for the body to cover the implant with a thin layer of scar tissue. Occasionally, an excessive amount of scar is formed and it contracts or shrinks around the implant making it feel unacceptably firm. If mild, the breast or breasts are moderately firm. If severe, the breasts can appear distorted and be hard and uncomfortable. This can be treated by a surgery to remove the scar tissue and replace the implants. It may or may not occur again.

Rippling: Being able to feel the implant due to a small amount of breast tissue to begin with OR losing weight after breast augmentation surgery.

Interference with mammography: All implants interfere with mammography but the implants placed under the muscle and therefore interfere less with mammography than implants placed above the muscle. We recommend patients 38 years or older get a baseline mammogram prior to the surgery.

Rupture/deflation: Breast implants do not last a lifetime. Some breast implants may rupture/deflate in the first few months after surgery and some after several years. Others may take 10 or more years to rupture/deflate.

Implant Exposure: Is usually associated with not enough tissue coverage for the implant as a result of an infection. Further surgery would be necessary.

Spreading of the areola may also occur due to pressure of the implant placed on the incision

Possible Risks and Compications of Breast Implant Removal

Asymmetry: Almost everyone’s breasts are different to begin with and Dr. Pousti will do his best to make them as symmetric as possible — we can never guarantee that they will be perfect.

Bleeding: blood loss in this surgery is minimal. It is important after surgery, especially in the first 2 weeks, to avoid exercise and raising your heart rate and blood pressure. Also avoid aspirin, ibuprofen and many other medications that may affect your ability to clot your blood.

Infection: this is very rare but can be taken care of with antibiotics if it occurs.

Breast Sagging / Unsatisfactory redraping of the skin: ptosis or sagging of the breast may occur if the breast tissue stretched as a result of the breast implants does not tighten up post-surgery.

Emotional / Physiological changes: it does take time to get used to the physical changes after breast implant removal surgery, especially for those who have had breast implants for quite some time.

Fluid Accumulation (seroma): Tissue fluid may build up in the area where the breast implant was removed. Additional treatment would be necessary to remove the excess fluid.

Contour Depression / Inversion of Nipple Areola Complex:

Unsatisfactory Results:

Displeasing breast size / shape: After breast implant removal, the breasts will not look the way they did prior to breast augmentation. It may take time to physically and emotionally get used to the loss of breast size / volume after implant removal.

Possibility of further surgery: Breast lifting or a breast re-augmentation may be desired after breast implant removal to either lift the breast tissue or add volume.
