| This 30-year old patient from Scottsdale, Arizona found Dr. Pousti when she was searching for the “best Plastic Surgeon” to assist her in correcting her symmastia. She had multiple conversations with Dr. Pousti on the phone and felt very comfortable with his experience level. She had had multiple surgeries performed without success and wanted to make sure that surgery #3 would work.



After seeing the results that Dr. Pousti had achieved for other patients in the same situation, this patient decided to proceed with revisionary breast augmentation surgery and symmastia repair.


The night before surgery, Dr. Pousti performed markings on the patient and also went over her revisionary breast surgery in detail again.
Symmastia – commonly referred to as “breadloafing” or “uni-boob”, occurs when two implants touch one another in the center of the chest. If the horizontal muscle that is connected to the sternum and goes across the implant is cut during surgery, then the implant can move toward the middle of the chest. Symmastia results from overly aggressive attempts to alter chestwall anatomy trying to increase cleavage in patients.


In the operating room, Dr. Pousti took his time marking the areas of implant mal-position. He wanted to correct the symmastia, improve the position of the breast implants and get a more symmetrical result for the patient. The “red markings” are where the internal suturing will be performed to separate the two breasts.

The repair of symmastia requires additional surgery to move the implant back into the pocket of original placement. All sutures are internal – there are no new scars placed on the breasts.
* The tissue that encapsulates the implant is cut and rolled back to fit into the pocket.
* Tissue is sutured together to hold implant in place.
* The muscle is then sutured internally back in place, connecting the sternum.
* The incision is then sutured close.

After the symmastia repair, temorpary in-sizers were placed and filled to to the patient’s desired look.

After the temporary in-sizers are filled, Dr. Pousti sits the patient up-right to check for desired size and symmetry. Then silicone-gel breast implants were selected.

Immediate post-op pictures were taken of the patient in the operating room with an amazing result.

These supportive bras are worn to give support to the area that was corrected with internal sutures. The “T-Bra” is worn for 3 months to continue to give support.
The patient is 5-days post-op surgery with 575cc silicone-gel breast implants with submuscular placement. She is now back in Arizona and doing well.
