| This 26-year-old patient from San Diego, California was post breast augmentation surgery in 2005 (performed by another surgeon). After several years, the patient started to notice some complications. As a result of her complication (symmastia) the patient started to research for a Board Certified Plastic and Reconstructive Surgeon who specializes in symmastia repair - revisionary breast surgery.

Symmastia - commonly referred to as “bread loafing” or “uni-boob”, occurs when two implants touch one another in the center of the chest Symmastia results are from overly aggressive attempts to alter chest wall anatomy trying to increase cleavage in patients. After reading many excellent reviews on the Internet, the patient decided to meet with Dr. Pousti at his San Diego office to discuss her surgery in detail. The patient was impressed by the number of successful revisionary breast surgeries performed by Dr. Pousti, the patient decided to proceed with surgery.

Markings and measurments were carefully made the night prior to surgery. The “red” marks are where the “internal bra” will be to separate the two breasts and improve the cleavage area. Dr. Pousti will place a double layer of internal sutures to help with better implant placement.

(Intra-Operative) The repair of symmastia requires additional surgery to move the implant back into the pocket of original placement. All sutures are internal – no other incisions were made on the outside of the breasts. The incision previously used for her breast augmentation will be used again for this revisionary breast surgery.

(Intra Operative) The tissue are sutured together to create in “internal bra” to hold implant in place. The muscle is then sutured internally back in place, connecting the sternum. The incision is then sutured close, and implants are submuscularly placed to provide some softening of the transition to the cleavage area from the augmented breast mound.

(Intra Operative) Surgical tape was used to reinforce the repairs made to ensure healing.

A thong bra and surgical bra is worn for about 2-3weeks before any dressing are removed.
The patient is now 1-month, post-op surgery with saline breast implants and filled to 330cc in both breasts, the patient is very pleased with the results of her surgery.
