Asymmetrical Constricted Breast Augmentation

This 23 year old patient from La Jolla, California, with an athletic body build was interested in breast augmentation surgery to correct her breast asymmetry and condition known as tuberous breast deformity. Breast asymmetry is a condition where one breast is noticeably larger than the other. Patients may start with different sized breasts before surgery (most patients are not perfectly symmetrical). Some cases of breast asymmetry are more severe than others, but there are several surgical options to promote better symmetry. Different types of implants may be used, different volumes may be used to fill up the breast implants, or changes in the pocket dissection may be used to help achieve as much symmetry as possible. Dr. Pousti takes his time in the operating room to make sure he achieves the patient’s goals. Dr. Pousti will determine which procedure will benefit each individual patient based on her concerns. The tuberous breasts deformity is when breasts have a very narrow base and usually a long skin envelope.

I am 5′11” 140 lb. graduate school student with size A asymmetric Tuberous Breast Deformity (constricted breasts). The asymmetry and shape of my breasts made me so self-conscious that, despite being a serious athlete (running and biking), there wasn’t one day in the last 10 years that I didn’t wear a padded bra. I am writing this review 3 weeks post-op and have never been more confident or happy with myself, all thanks to Dr. Pousti (see my separate review of Dr. Pousti below). I received silicone implants, 465 cc in the left and 435 cc in the right. My incisions were areolar to perform the lift and construction necessary to fix my Tuberous Breast Deformity.

His staff members are friendly, supportive and excellent at what they do. Since my surgery 3 weeks ago they have been in constant contact with me; making sure I have all of the medications I need, checking in to see how I’m feeling, answering all of my questions, and scheduling follow-up visits. Tuberous Breasts have a very narrow base and usually a long skin envelope. In the most severe cases of Tuberous Breast, a breast lift can be done through an incision around the areola, making the breast into a more rounded shape that the patient will be happy with. In many cases however, a lift is not necessary. The patient should be aware that the final result will take months to see and that they will need to be patient.

This same patient came back and after several years of having breast implants and significant consideration, she decided to have her breast implants removed. The patient also desired to make the size of her areola smaller.

This 25 year-old patient from Oceanside, California was interested in breast augmentation surgery for several years.  Unhappy with her “tubular breasts” and breast asymmetry she wanted to have surgery to correct it. Tuberous Breasts have a very narrow base and usually a long skin envelope. She researched for Board Certified Plastic Surgeon herself, and came across Dr. Pousti on the Internet, and after look through many before and after pictures of patients he has help with the same case the patient knew Dr. pousti would be the best surgeon for her.

During her consultation, Dr. Pousti and the patient discussed improving symmetry and shape to  achieve her desired look, a limited breast lift would be needed for both breasts.  To perform a benelli lift, Dr. Pousti would place the incision around the areola (with time, this incision heals beautifully and is barely visible)

This 22-year old patient from Downey, California was interested in enhancing her breasts with Breast Augmentation Surgery for several years.  She wanted to significantly increase her breast size.breast augmentation

The patient wanted a surgeon that is Board Certified in Plastic and Reconstructive Surgery and found Dr. Pousti on the Internet. During the consultation Dr. Pousti informed her that she had a mild case of  tuberous (tubular) breasts deformity. In order to achieve her desired look, the “constriction” would have to be addressed to achieve a more desirable and long term result.

The lower pole of the breasts needed to be “rounded out” and the distance from the areola to the infra-mammary fold needed to be increased.  Dr. Pousti worked hard in the operating room to achieve nice results for this patient.

Patient was carefully measured and marked prior to surgery. You can see that the left breast is 2 cm lower than the right breast.  This breast asymmetry will be improved with the breast lifting / augmentation surgery.

Dr. Pousti uses the Benelli incision around the areola to release constricted breast tissue, also removing the excess skin envelope correcting the projecting areola..  600cc on right and 595cc on left silicone-gel breast implants made by the Sientra Corporation in both breasts with submuscular placement. The patient is very pleased with the results of her surgery and new and improved look.

A 27 year old female from Sherman Oaks, California wanted to enhance her breast for years.  She was unhappy with the shape and size of her constricted breast, also known as Tuberous Breast. Tuberous Breast Correction

A 22 year old woman from Canada was interested in bilateral breast augmentation and correction of bilateral tuberous breast for several years . Tuberous breasts have a very narrow shape and usually a long skin envelope, and usually an incision around the areola  to help make the breast have a more rounder shape.

The patient was unhappy not only with the size of her breasts but also with the shape and the breast asymmetry.  She researched to find the most experienced plastic surgeon to help her with her tuberous breasts and found Dr. Pousti online.  After viewing his before and after gallery, she decided to contact him and have a virtual consultation.  They communicated several times and finally met the day prior to surgery.

Dr. Pousti performed measurements and markings prior to surgery.  As  you can see above, the distance from the collar bone to the areola on each side differs.  This will be addressed during the surgery and Dr. Pousti will do his best to improve the breast symmetry.  Also, the side with the “smiley face” indicates the side with more breast tissue.  All of this is taken into consideration when choosing the implant size, performing the pocket dissection, and performing the benelli  lift.

Dr. Pousti preformed Bilateral Breast Augmentation with Silicone Gel Implants and Correction of Bilateral Constricted Breast to achieve symmetry.  In her case a full breast lift was not needed – only an incision around the areola was needed to achieve the patients goals.  You can see the significant improvement in breast size, shape and symmetry.  The patient is very pleased.

Breast Surgery for Correction of Tuberous Breast

This 29 year old from Georgia was interested in a breast augmentation for several years. She found Dr. Pousti online after searching for an experienced board certified plastic surgeon. After exchanging several emails and telephone calls, the patient felt the trip to California would be well worth it to have Dr. Pousti perform her breast enhancement surgery so she flew into San Diego to meet with Dr. Pousti in person.

Tuberous Breast

Dr. Pousti examined the patient and determined she was a good candidate for a breast augmentation with correction of tuberous breasts as well as breast asymmetry.  Tuberous or constricted breasts, usually have a very narrow base and long skin envelope. The areola can also appear puffy or enlarged. Dr. Pousti discussed the pros and cons of saline vs silicone gel implants and the patient decided to proceed with a silicone gel breast augmentation surgery with correction of her tuberous breast.

Tuberous Breast

The patient and Dr. Pousti met again the night prior to surgery to review the planned procedure in detail. During this appointment, measurements and markings are also performed. Markings are key in achieving as much symmetry as possible.

Markings are checked several times prior to the start of surgery. The patient and Dr. Pousti met again in the pre-operative room at the surgery center to reinforce markings and make any necessary changes needed.

Tuberous Breast

Dr. Pousti used sizer implants to determine what size implant was needed to achieve the patient’s ideal breast size and shape. The sizer implant was placed into the breast pocket and filled with air until the patient’s goal size was achieved. Once sizing in noted, the temporary implant was removed and the silicone gel implant was prepped for placement using the Keller Funnel method. This method uses a sterile funnel to easily place the breast implant into the breast pocket.  For this patient, the incisions were placed around the areola for improvement of areola asymmetry as well as correction of the “tuberous” breast.

Constricted Breasts

After placing the implant, Dr. Pousti performed correction of the patient’s constricted breast. This operation involves removing the fatty tissue of the areola, reducing the puffy areola.

Dr. Pousti removed fatty tissue from both the left and right breast areola.

A 350cc silicone implant was used on the right breast and a 325cc silicone gel implant was used on the left breast. Slightly different size implants were used to achieve as much breast symmetry as possible. The patients breast pockets were lowered to allow room for the breast implants and to give the patient a more rounded look, which is common with tuberous breast patients.
