Overfilled Saline Breast Implants Patients

| This 24 year old patient from Livermore, California had been interested in revisionary breast surgery after she was left with unsatisfactory results from her previous surgeon. After doing further research she came across board certified plastic surgeon Dr. Pousti and was impressed with his experience with breast revision surgery and decided that it was worth the travel to have him perform her surgery.
The patient was also interested in extra large implants and was having difficulty finding surgeons willing to go over 800ccs.
Prior to her surgery the patient and Dr. Pousti meet several times to discuss revisionary options and the breast procedure in detail. The last appointment before surgery measurements of the patient’s breasts are taken and markings are made. The markings are rechecked again in the operating room and adjustments are done if needed.
For the first step of the revisionary breast surgery the patient’s previous breast implants are removed.
Once the implants are removed Dr. Pousti revises the breasts pockets for the new implants. The patient had expressed that she was unhappy with the appearance of her breasts while she was lying down. To fix this Dr. Pousti used internal sutures to prevent the implants from falling to the sides. Sizers are then used to determine the size of implant needed to meet the patient’s size goals.

| This 21 year old patient from Lakeside, California was interested in removing her current implants and replacing them with much larger implants. She decided to proceed with Dr. Pousti for her breast procedure after finding out that he was very experienced with plastic and reconstructive surgery. She had procedures performed by him before and felt very comfortable with his work.
During her visit with Dr. Pousti extra large breast augmentation options were discussed. Upon examination, Dr. Pousti felt it would be in the patient’s best interest to also have her breasts lifted during her surgery to achieve an overfilled perky look.
The patient decided to continue with breast re-augmentation and breast lift. The patient met with Dr. Pousti to take measurements and perform markings so symmetry would be achieved.
Below, it is very apparent that the right breast is “perkier” than the right after lifting and replacing the implant. The right breast is temporarily stapled until comparisons between the two breasts can be made and symmetry is achieved, following final sutures.
Dr. Pousti used Smooth Round Moderate Plus Profile Saline Implants filled to 1050cc on the right and 1100cc on the left. The left breast was overfilled slight larger than the right to achieve symmetry.
The patient is very happy with her breast lift and extra large breast augmentation results.

| This 37 year old from Australia was interested in revisionary breast surgery. After being left unsatisfied with both of her previous breast augmentation surgeries, she wanted to extensively research prior to choosing a surgeon. Not only did she research cosmetic surgeon’s, but she read several testimonials from patients who had revisionary breast surgery. She found that she kept reading reviews from patient’s of Dr. Pousti’s and how satisfied they were with their breast surgery results. The patient felt strongly after contacting Dr. Pousti, that the trip from Australia to California would be well worth it to have her surgery performed by Dr. Pousti.
Upon arriving in California, the patient met with Dr. Pousti and discussed all of her options. The patient was interested in removal of her current breast implants and replacing them with Saline Overfilled implants. She wanted an emphasis on outer fullness with minimal fullness on top. Dr. Pousti and the patient agreed Saline Overfilled implants would give the patient the look she desired.
Dr. Pousti and the patient proceeded to the surgery center after completing measurements and markings to identify exactly what would be done during the surgery. Markings are extremely important and guide Dr. Pousti through the surgical process. They also are able to paint a picture for the patient as to what areas will be addressed during the procedure.
Dr. Pousti used Smooth Round Moderate Plus Profile Saline Implants filled to 1030cc on each breast. After the implants have been placed under the muscle, Dr. Pousti will sit the patient in the upright position to ensure that a desirable look has been met. The patients satisfaction is one of Dr. Pousti’s main concerns along with safety.
The patient is now 3 weeks post op and is very satisfied with her Saline overfilled implants.

| This 27 year old from Livermore, California was interested in Breast Revision Surgery after being left unsatisfied with her previous Breast Augmentation procedure by another surgeon. She was not happy with the shape and size of her breasts as well as the placement of her implants. She also complained of the implants falling to the side when lying down, which bothered her as well. The patient wanted fuller and rounder implants with more cleavage concentration towards the mid line of her chest. She was researching the internet for a Board Certified Plastic Surgeon with extensive experience in revisionary breast procedures when she found Dr. Pousti’s website. His credentials and albums of “before and after photos” containing cases similar to hers, helped the patient decide the trip from Northern California to Southern California would be well worth it.
After communicating through emails and phone calls, the patient arrived in San Diego to discuss her options with Dr. Pousti. He felt that she would benefit from revisionary breast surgery as well as capsulorrhaphy, to correct the position of her implants while lying down. The patient felt comfortable with proceeding to surgery with Dr. Pousti. Markings were done prior to surgery and then re -checked before beginning the procedure to make changes as needed.
Dr. Pousti exchanged the patients previous implants with new overfilled Saline implants filled to 1530cc on each breast to give the patient her desired fullness. Capsulorrhaphy, also known as pocket correction or the use of internal sutures or an internal bra, was also performed. This procedure includes closing off a portion of the breast pocket to fix displacement of the implant, in this case to fix the problem of the patient’s implants falling off to the side.

| The 23 year old patient from Colorado Springs, Colorado was very unhappy with the results of her breast augmentation surgery, which was performed by a surgeon near her hometown. She decided to look into plastic surgeons that were experienced with breast augmentation revisionary surgeries and came across Dr. Pousti’s website.
After traveling to California and meeting with Dr. Pousti she immediately felt comfortable with him and decided to proceed with revisionary breast surgery. The patient met with Dr. Pousti quite a few times before her procedure to review the breast revision in detail.
Markings of the patient’s breasts are performed prior to surgery to indicate changes that are needed. The red arrows marked indicate where the patient wants more fullness and helps Dr. Pousti determine where the breast pocket revision needs to take place. Once these markings are completed the previous breast implants are removed through small incisions along the areola (usually the same incision that was originally used will be used again to prevent more scars).
The patient is placed in an upright position several times during the procedure to ensure symmetry is achieved. Once the old implants are removed and breast pocket correction is performed, sizers are set in place. These sizers help determine the size of the implant Dr. Pousti will use and also help him see what the potential results will look like before anything is finalized.
To meet the patient’s size goals and meet her desired look Saline Overfill implants were used. The patient has 1,110 cc on both sides.
The patient is a extremely happy with her revisionary breast surgery results.

| This 30 year old patient was interested in Body Contouring Surgery after massive weightloss. She had lost 80lbs after having gastric bypass surgery and was interested in revisionary breast surgery along with abdominoplasty. She wanted an extra large, overfilled implant.
This patient is from Austin, Texas and after doing extensive research online decided that the travel to San Diego, California to see board certified plastic surgeon Dr. Pousti, was well worth it.

| This 29 year old from Portland, Oregon had been unhappy with her breast size and the “droopiness” for some time now and was interested in getting breast implants. | This 30 year old Mom of 1 from Temecula, California (Riverside County) had lost a significant amount of weight and was wanting to improve herself before getting married this fall. She wanted a mommy makeover with breast surgery as well as tummy tuck surgery.
She wanted more fullness in her breasts and wanted all of the excess skin and adipose tissue from her abdomen and flanks removed. She had lost over 135 pounds just be eating right and exercising and she was ready for a change.
She and her family searched online for a Southern California Plastic Surgeon with experience in performing mommy makeovers (breast augmentation, breast lifting and abdominoplasty surgery). They found Dr. Pousti and communicated via email and then came in for the consultation. She and Dr. Pousti took a look at her skin elasticity as well as her current breast tissue and her goals to be “much bigger” and decided to proceed with breast augmentation with overfilled saline implants, benelli breast lift and extended abdominoplasty (tummy tuck that extends into the back with a flank lipectomy).
Dr. Pousti performed markings the morning of surgery. The markings involve Dr. Pousti taking measurements, drawing on the patient as to how the procedures will take place and going over the procedures in detail with the patient one more time.
The measurements are checked and re-checked prior to surgery. You can see below all of the measurements on the patient’s back. Dr. Pousti takes great care to remove as much as possible without jeopardizing the patient’s recovery.

Here you can see the upper abdominal muscles which will be sewn together (as they have been on the lower part) to give the patient that “tight” abdomen she was wanting.
In the operating room, you can see the breasts and tummy being worked on.
Immediately after surgery in the operating room… with fuller, larger breasts and flat tummy. She had breast implants, minimal breast lift around the areola and extended abdominoplasty surgery.

| This 40 year old patient from Kingston, Canada pleased with her previous breast revisionary surgery but she is interested in XL implants and even more fullness if possible.
As a previous patient of Dr. Pousti she was very please with his work. She decided he would be the doctor for her breast revision procedure. She therefore scheduled a flight to San Diego after she and Dr. Pousti discussed all of the potential risks and complications with larger breast implants and more revisionary surgery. The patient understood the risks and wanted to proceed with surgery.
Before surgery Dr. Pousti makes markings to indicate the patient’s “goals” of more fullness / roundness. Again, Dr. Pousti discusses the specific procedure with the patient and her husband in detail. They proceed to the operating room.
 Dr. Pousti removed her breast implants and replaced them with new saline breast implants over-filled to 1740 cc in both breasts to achieve the “look” the patient wanted.
The patient is 5 months post op and is extremely happy with her results. She continues to follow up with Dr. Pousti via email and phone and will be sending updated photos via email.
