The internal finish, containing an oily hydrophilic coating, permits surgeons to naturally drive the implant to the breast pocket with insignificant power and decreased force on the implant as well as the incision. The implant is positioned in the opening of the funnel with the petite end placed in the incision leading to the breast pocket and a simple squeeze allows the implant to make its way through the funnel and into the breast pocket.

The convenience of the Keller Funnel allows Dr. Pousti to administer the implant with a no contact method, allowing for decreased risk of contamination, less finger weakness during the procedure, as well as reduced surgery time. With the use of the funnel, Dr. Pousti is permitted to make a minuscule incision length in a variety of areas, resulting in quicker healing and improved comfort for the patient as well as less tissue damage.

Preparing For Surgery

San Diego Plastic Surgeon Dr. Tom Pousti is a double board certified surgeon certified in General Surgery as well as Plastic and Reconstructive Surgery serving the San Diego and Riverside County with over 20 years of broad medical and surgical training experience. Dr. Pousti is known for his warm, caring, and respectful approach to his patients as well as his extraordinary results. Before each surgery, Dr. Pousti takes the time to meet with you in the office for your pre-operative appointment, usually the 3 weeks before surgery. Another appointment would be the night prior to surgery, during this appointment, Dr. Pousti will perform “markings”. These markings and measurements aide in preparing Dr. Pousti as well as the patient for the upcoming surgery. Dr. Pousti identifies each patient is unique with diverse body types and structure, which is why marking are tremendously important to him and aide the patient in understanding what exactly will be addressed and focused on during the operation. Markings reassure and that each surgery is tailored to that patient’s specific needs. Measurements are always carefully made to achieve the desired goal of the individual patient. Markings are also reinforced the morning of surgery, at the surgery center, to help reinforce that the desired goal is met.

Abdominoplasty (Tummy Tuck) Markings:

In the surgery center, Dr. Pousti is re-checking all the measurements and defining markings one last time before surgery starts to ensure the best results.

The muscle repair has already been finished and Dr. Pousti is showing exactly how much skin he removed from this particular patient. Once he removes the rest of the excess skin, he will bring the belly button through a new hole and finish suturing the skin together.

Augmentation Mammoplasty (breast augmentation) markings:

Dr. Pousti is measuring this patients breasts to ensure that symmetry as well as fullness is achieved following surgery.

Here, Dr. Pousti shows the patient how one areola will need a slight lift with the augmentation to achieve the best results.

The patients markings and measurements have been reinforced prior to surgery.

This is the patient immediately after surgery. Dr. Pousti has the patient in an upright position to double check symmetry and placement to guarantee remarkable results.

Mastopexy (breast lift) markings:

Dr. Pousti uses these markings to assist in the surgery room during this procedure which involves rearranging breast tissue, altering the nipple location, and amplifying the breast firmness by tapering the skin.

Measurements for this procedure help Dr. Pousti with symmetry and location of the breasts following the surgery.

After examining the measurements and markings, the patient will be prepared for surgery.

Dr. Pousti observes the results before finishing the procedure.

Reduction Mammoplasty (breast reduction) markings:

Dr. Pousti uses markings for this particular procedure to reduce and reshape the appearance of the breast.

Measurements are also made to establish symmetry during the surgery.

The markings are determined by what technique will be used during the surgery, which is dependent upon the patient’s age and anatomy.

Dr. Pousti shows the initial results of breast reduction performed on one breast.

Revisionary Breast Markings:

Dr. Pousti is marking the red area, the area of concern, to demonstrate which area will be fixed during the procedure. The red indicates where the implants are touching the center of the chest creating a “uni-boob”.

The doctor examines the patient in the operating room again prior to the start of surgery.

Directly following surgery, the breasts are no longer touching the center of the chest with the aide of pocket correction and internal sutures.

Brachioplasty (arm lift) markings:

Markings of the excess skin and fat are being applied to help Dr. Pousti and the patient prepare for surgery.

Dr. Pousti is finishing the markings on the area of skin that will be removed. Exact placement of markings are important to conceal the incision and future scar.

Chest Wall Reconstruction Markings:

Dr. Pousti is using a moulage is being used to mold a silicone pectoral implant.

Dr. Pousti is marking the sunken area to ensure exact placement of the silicone implant.

The markings represent the area of the sunken chest where the implant was placed to fill out the dent.

Pectoral Implant Markings:

Dr. Pousti measures this patient prior to surgery to guarantee projection of the chest wall focusing on the pectoral region.

An incision had been made by Dr. Pousti where he will insert the implant.

Dr. Pousti reveals the initial results and differences with one pectoralis implant placed.

Blepharoplasty (eyelid surgery) Markings/Brow Lift (forehead lift) Markings:

Dr. Pousti is applying the markings on the eyelids where he will reduce the bulging and wrinkling of the skin. He also creates markings on the forehead to remove or rearrange the muscles and skin that can often cause the wilting or droopiness.

The marking have been re-checked by Dr. Pousti and now the patient is ready for surgery.

The patients forehead and eyelids have improved significantly and appear rejuvenated.

Otoplasty (ear surgery) Markings

Dr. Pousti performs markings to fix the surplus prominence of the ears.

Markings are checked and re-checked for flawlessness.

Dr. Pousti’s technique has fixed the projecting of the ears.

Liposculpture (liposuction) Markings:

The markings represent the area that will be treated with liposuction.

Dr. Pousti is performing liposuction to exonerate stubborn fat.

Labia Reduction Markings:

Marking are applied by Dr. Pousti to remove excess skin, however avoiding an interference with the urethral opening and clitoris are important.

Description

While breast augmentation or mastopexy surgery is relatively simple and generally complication free, breast augmentation/mastopexy surgery done together carries increased chances of complications and need for further surgery. For example, there is an increased risk of infection, implant exposure, breast asymmetry, loss of nipple-areola sensation, inability to breast feed, mal-positioning of the nipple-areola complex, mal-position of the implants, wound healing problems, tissue necrosis, loss of blood supply to the nipple-areola complexes. Any of these complications may require further surgery, therefore, increasing the likelihood of revisionary surgery. To avoid these risks Dr. Pousti frequently recommends a ‘2-stage procedure’. First stage being a breast lift followed by augmentation once the patient heals. Generally good candidates for a 2-stage procedure are patients that need a significant amount of skin removed or patients interested in extra large implants.

The Surgical Procedure

Prior to the procedure the patient is examined by Dr. Pousti. The planned procedure of the first stage of the procedure (the mastopexy) is explained in detail.

The markings of the breast lift surgery are done the day before in the office. During the mastopexy the nipple-areola complex is relocated, the breast tissue is rearranged and the breast placement is improved by tightening the skin.

After Patient has Healed from Breast Lift

Once the patient is healed it is time for the second stage, the breast augmentation. Markings of the breast enhancement procedure are done prior to surgery as well.

The implants are placed in the breast through a small incision. In most cases Dr. Pousti will use the same incision site used for the breast lift to prevent further scaring.

What To Expect After Surgery

Pain is rarely strong, more commonly being a degree of discomfort. Swelling occurs but usually begins to subside by the third or fourth day. Some degree of swelling may persist for longer periods. A well-fitted bra is worn day and night for three weeks. While there is probably seldom interference with future breast feeding, women are cautioned that it may not be possible to breast feed in the future.

Resumption Of Physical Activities

Driving may be resumed in 2 weeks.
Non-contact sports in 3 weeks.
Contact sports in 6 weeks.
Sexual activity in 2 weeks.
